• A face mask is required during your entire visit
• Patient may have (1) adult visitor with her during her office visit (must be vaccinated and show proof of vaccination upon request).
• No children are allowed into the office

• Please reschedule your appointment if you are feeling unwell.
• Please be on time to your appointment to avoid an overcrowded waiting room.
• We ask that you bring only one person with you to your visit if needed, to minimize overcrowding.
